The Danish Headache Questionnaire (DHQ)
General
The Danish Headache Questionnaire (DHQ) includes a survey that covers diagnostic procedures, management strategies, and treatment modalities, and a logbook for tracking the prevalence of consultations for headaches. The purpose of the survey is to measure clinician characteristics and knowledge when managing patients with headache and to obtain accurate headache prevalence estimates in clinical practice.
The survey is a 47 item questionnaire measuring 10 domains:
- Practitioner demographics
- Knowledge of the Danish profession-specific clinical care standard
- Knowledge of the categorisation and diagnosis of primary types of headaches
- Knowledge of the categorisation and diagnosis of secondary types of headaches
- Knowledge of the classification and diagnosis of other types of headaches
- Medical history, physical examination and x-ray
- Monitoring measures
- Multidisciplinary care
- Treatment modalities and outcomes
- Number of consultations, duration, and frequency of care
- Perception of efficiency
- Side effects

Terms and conditions
Licence
The DHQ questionnaire and accompanying logbook may be used free of charge for non-commercial scientific or clinical purposes, provided they are administered as described in the original development article and, for English-language applications, in accordance with the DHQ User’s Guide. The instruments may not be modified, edited, shortened, expanded, translated, or otherwise altered without prior written permission from the copyright holder, and all uses must cite the original publication(s) appropriately.
Definition of commercial use
Any use that is primarily intended for, or results in, direct or indirect financial gain or competitive advantage. Examples include:
- Incorporating the questionnaire or logbook into software, apps, books, courses, devices or other products that are sold, licensed or subscription-based.
- Use by private clinics or other health-care providers when administration is billed to patients, insurers or sponsors.
- Use by contract research organisations, pharmaceutical or med-tech companies, or other for-profit entities in sponsored studies, product development or marketing.
- Redistribution, translation, adaptation or modification of the DHQ or logbook for sale or licensing.
Permitted non-commercial use (illustrative)
- Publicly funded or grant-funded academic research.
- Routine outcome monitoring in publicly financed health-care settings where no separate fee is charged for the questionnaire.
- Teaching and training in universities or other non-profit educational environments.

Translation and cross-cultural adaptation of the DHQ and logbook
If you wish to translate and cross-culturally adapt the DHQ and logbook into another language, please send a request to Kristina Dissing on e-mail: kbdissing@kiroviden.sdu.dk. Translation and cross-cultural adaptation of the DHQ and logbook uses the English version (see below), and must follow the guidelines outlined by Beaton et al. → abstract.

When translation and cross-cultural adaptation has been completed, we ask that all the documentation and the final translated version are forwarded to Kristina Dissing.
We collect and publish all translated versions of the DHQ and logbook on this website to avoid multiple translations of the survey in a specific language.

The Turkish generic version of the DHQ is underway and will be available in due course. It will be translated and cross-culturally adapted using the Beaton et al.
